Medicare in California

Medicare in California: A Complete Guide

Choosing Medicare coverage in California means understanding three things that vary from state to state and even county to county: which Medicare Advantage plans and carriers are actually available where you live, how California's Medigap rules affect your ability to buy and switch a supplement, and how the Part D drug landscape looks. This guide covers all three, honestly, from a licensed independent broker who earns the same regardless of which plan you pick.

The Medicare landscape in California

California is the largest Medicare market in the country and one of the most competitive, but it is really a collection of very different regional markets. Kaiser Permanente is dominant in much of the state and consistently earns top star ratings, though it only works if you are comfortable using Kaiser doctors and hospitals. Around it, SCAN, Health Net, Blue Shield of California, Anthem Blue Cross, and Alignment compete hard, and what wins comes down to your county and your providers. California's Medigap Birthday Rule is a genuine advantage worth understanding, because it lets you shop your supplement every year.

Medigap rules in California

Birthday Rule state

California has a Medigap Birthday Rule, one of the most useful consumer protections you can have. Each year around your birthday you get a window in which you can switch to another Medigap plan of equal or lesser benefits with the same or a different company without going through medical underwriting, meaning you cannot be turned down or charged more for your health during that window. The key limits are that the new plan must offer equal or fewer benefits than your current one (you cannot use the birthday rule to upgrade to richer coverage), and the window is time-limited, so you have to act inside it. This lets healthy and unhealthy beneficiaries alike shop their Medigap premium every year and move to a cheaper carrier for the same coverage. The exact length and start of the window can change, so verify the current rule before you count on a specific date.

Whichever plan you land on, the two most popular Medigap choices nationwide are Plan G and Plan N, and our guide on Plan G vs Plan N breaks down the trade-off. To understand exactly when an insurer cannot turn you down, read our complete guide to Medigap guaranteed issue rights. State rules do change as legislatures update them, so verify the current rules for California before you rely on a specific window.

How enrollment works in California

The enrollment windows in California are the same federal windows that apply everywhere, so the calendar is the part that does not change by state. When you first become eligible, usually around your 65th birthday, you get an Initial Enrollment Period that runs for seven months: the three months before your birthday month, the month itself, and the three months after. That is your cleanest chance to enroll without penalties.

Every fall, the Annual Enrollment Period runs from October 15 to December 7, and this is when you can join, switch, or drop a Medicare Advantage or Part D plan for the following year. If you are already in an Advantage plan, the Medicare Advantage Open Enrollment Period from January 1 to March 31 gives you one more chance to change Advantage plans or return to Original Medicare. Outside those windows, a Special Enrollment Period may open if your life changes, for example if you move, lose other coverage, or qualify for extra help.

If you are approaching 65 in California, the smartest move is to map out your specific timeline before the deadlines arrive. Our month-by-month turning 65 checklist walks through exactly what to do and when.
